Physiology-Informed Precision Care for OSA: Moving Beyond CPAP

In this insightful webinar the renowned sleep researcher Dr. Danny Eckert Director, Adelaide Institute for Sleep Health at Flinders University. Explores the paradigm shift toward physiology-informed precision care for Obstructive Sleep Apnea (OSA).

If your dental practice treats sleep-disordered breathing, you likely know that the traditional “one-size-fits-all” trial-and-error approach, where most patients are initially prescribed CPAP, which leaves many patients non-compliant and untreated. In this presentation, Dr. Eckert introduces the PALM scale, a framework that categorizes the four specific traits (endotypes) driving a patient’s sleep apnea: Pcrit (anatomical collapsibility), Arousal threshold (how easily they wake up), Loop gain (respiratory control stability), and Muscle responsiveness.

By understanding that OSA is a heterogeneous disorder, global practitioners can better tailor their treatments. Key highlights from the webinar include:

  • Predicting Treatment Success: Discover why patients with a low arousal threshold (light sleepers) often struggle to tolerate CPAP therapy. Conversely, learn how patients with a moderately collapsible airway and stable non-anatomical traits are likely to respond favorably to Oral Appliance Therapy (OAT).

  • The Power of Combination Therapies: Dr. Eckert shares research demonstrating how “failed” oral appliance cases can often be resolved by adding targeted therapies, such as positional devices to keep patients off their backs, or emerging pharmacotherapies, effectively treating most patients without needing CPAP.

  • Advanced Multi-Night Diagnostics: Understand the limitations of single-night diagnostic sleep studies, which can misclassify up to 50% of mild cases due to night-to-night variability. Explore how multi-night tracking technologies (like under-mattress sensors) yield more reliable data for identifying cardiovascular risks like hypertension.

  • Clinical Translation: Learn how to utilize standard parameters from an existing sleep study (like the ratio of hypopneas to apneas) to estimate a patient’s anatomical collapsibility and arousal threshold, helping you and your collaborating sleep physicians make better treatment decisions today.

Watch the full presentation to elevate your understanding of sleep physiology, build stronger multidisciplinary referral networks, and deliver personalized, effective sleep apnea care to your patients.
